First, you need an iPhone (obviously). Remember: the iPhone must be jailbroken!
Second, you need iFunBox (free download at http://www.i-funbox.com/ ). You can’t do it without this app. It is a Windows program and there are Mac versions for Lion and Snow Leopard on their website as well. Just search for “iFunBox” in the App Store.
Third, you need a .ipa file. Just search it up in your favorite search engine and download the first torrent/download you find. This must be an offline installer! The original app store apps are usually updated on a daily basis, so they can be installed directly over the air (OTA). If you want to hide your track, you have to use an app that is not updated often.
Open iFunbox and connect the iPhone with the USB cable. The app will recognize the iPhone automatically and show it in the left panel. Now, click on Applications.
It is very important for you to make sure that “cache applications” is checked in the options, otherwise you’ll get an error message that the app cannot be installed.

At this point you are almost finished; just click on “Install Cached Apps” and navigate to the .ipa file. You will have to wait some time, maybe a few minutes. The app will be installed (you’ll see it in iTunes as if you would sync it) but keep in mind that the app is not actually running yet.

Look at all those folders! What we need is in /var/mobile/Applications/. Therefore, the last step is to navigate there in iFunbox and click on the app. This will create an additional folder with the name of the app’s bundle id.

In order to run the app, you have to copy/paste the .app folder from that bundle id folder into /Applications.
Congratulations! Now you can open the app on your iPhone and nobody will know how it was installed!
